Overview

The Gravity Forms NPS Survey Add-On makes it easy to measure customer loyalty and satisfaction directly through your WordPress forms.

Add a dedicated NPS field to any form to ask how likely someone is to recommend your business, product, or service. Respondents select a score from 0 to 10, and the add-on automatically classifies each response using the standard Net Promoter Score categories: Detractors, Passives, and Promoters.

Choose from four scale styles, customize the question and endpoint labels, and optionally select a color that complements your form design. Because the field works with Gravity Forms conditional logic and merge tags, you can personalize confirmations and notifications or display follow-up questions based on the submitted score.

Once responses begin arriving, view the calculated Net Promoter Score from the form’s NPS Survey results page. Visual reports show the response mix, category totals, score distribution, and NPS performance over time—helping you move beyond individual responses and identify broader trends.

When results need to be shared, generate a secret live-report link that displays aggregated NPS data without requiring a WordPress login. Links can be rotated or disabled whenever access needs to change, and shared reports can be printed or saved as PDFs.

The result is a focused way to collect, analyze, and share Net Promoter Score feedback without sending respondents to a separate survey platform.

How It Works

Edit a Gravity Form and add the NPS field from the Advanced Fields section. The field starts with a familiar recommendation question:

How likely are you to recommend us to a friend or colleague?

The field provides a fixed scale from 0 to 10. You can change the question, edit the labels at the left and right ends of the scale, choose a visual style, and optionally set a custom selected color.

When someone submits the form, the add-on:

  1. Validates and saves the selected score as a number from 0 to 10.
  2. Assigns the response to its standard NPS category:
    • Detractor: 0–6
    • Passive: 7–8
    • Promoter: 9–10
  3. Makes the score available to Gravity Forms conditional logic.
  4. Makes both the score and category available through merge tags.
  5. Displays the score and category in the entry list and entry details.
  6. Includes the response in the form’s aggregated NPS reporting.

Open Entries → NPS Survey to review the results. For each NPS field, the report displays:

  • The calculated Net Promoter Score
  • The total number of responses
  • Promoter, Passive, and Detractor counts and percentages
  • A visual breakdown of the response mix
  • NPS performance over time
  • The distribution of responses across every score from 0 to 10

Reports use weekly periods for shorter timelines and roll results up by month when the reporting history extends beyond 12 weeks.

If the form contains multiple NPS fields, you can search for a survey, jump directly to its report, or expand and collapse reports to focus on the results that matter.

Build Smarter Follow-Up Experiences

The NPS field works with Gravity Forms conditional logic, allowing you to adapt the rest of the form to a respondent’s score.

For example, add a Paragraph field and display it when the NPS score is less than 7 to ask Detractors what could be improved. You could also show a testimonial request, referral invitation, or review prompt to Promoters who select 9 or 10.

This makes it possible to collect the headline NPS metric and the qualitative feedback needed to understand it within the same form.

Use Scores and Categories in Notifications

The standard field merge tag returns the submitted numeric score, making it easy to include the result in notifications, confirmations, integrations, or other automated workflows.

Add the category modifier when you want to output the corresponding Promoter, Passive, or Detractor label instead. This can help internal teams understand the response immediately without interpreting the number themselves.

Share Live NPS Reports

Use the Share report controls on the NPS Survey results page to generate a secret report link. Anyone with the URL can view a live HTML report of the form’s aggregated NPS results without logging in to WordPress.

The shared report includes the overall score, response mix, category breakdown, NPS trend, and score distribution. It does not display individual entries or contact details.

Shared reports update as new form entries are submitted and include a Print / Save as PDF option for creating a point-in-time copy.

For access control, administrators can rotate the secret link to replace the existing URL or disable sharing entirely. Because anyone with the link can view the aggregated report, the URL should be treated like a password. project, and optionally connect the record to a FreshBooks client.

This section is a short usage guide for adding an NPS field, reading the score, and sharing the report. Requirements and development notes are covered outside this section.

Add an NPS field

Edit a form and add NPS from Advanced Fields. The scale is a fixed 0–10 list and is not editable.

Default copy when you add the field:

  • Question: How likely are you to recommend us to a friend or colleague?
  • Left label (0): Not at all likely
  • Right label (10): Extremely likely

In the field settings, set the label, description, required state, scale style, and the left/right scale labels. Selected color is optional: leave it empty to use the form theme color, or pick a color for the selected score and keyboard focus ring on Buttons, Circles, and Radio buttons.

The stored value is an integer 0–10, or empty when the field is optional and unanswered. Entries list and entry detail show the score plus its category, for example 9 — Promoter.

Choose a scale style

The scale uses Orbital form theme tokens (primary color, radius, borders, and type). Four styles are available:

Style Look
Buttons Numbered tiles. Selected score uses the form primary color, or a custom selected color when one is set.
Circles The same tiles, round.
Radio buttons Horizontal Orbital radios with the score under each.
Spectrum A connected 0–10 bar from red (detractor) to green (promoter).

Spectrum keeps its own red-to-green colors. A custom selected color does not change that bar.

Categories

Categories are the standard NPS buckets and are not configurable in this version:

Score Category
0–6 Detractor
7–8 Passive
9–10 Promoter

Merge tags

Use the field merge tag for the numeric score:

Add the category modifier for the promoter / passive / detractor label:

Conditional logic and notifications also use the numeric score.

Optional follow-up

This add-on does not include a built-in follow-up textarea. Add a Paragraph field and show it with conditional logic when the NPS score is less than 7 (detractors, 0–6). Gravity Forms uses is, is not, greater than, and less than for this field. The same pattern works for promoters (greater than 8).

View results

Forms that contain an NPS field get Entries → NPS Survey in the Gravity Forms toolbar.

The Results tab shows, for each NPS field:

  • Net Promoter Score (% promoters − % detractors)
  • A stacked mix bar for promoters, passives, and detractors
  • Promoter, passive, and detractor counts
  • NPS over time (weekly, or monthly when there are more than 12 weeks)
  • The 0–10 score distribution, colored by category

Date filters on that screen come from Gravity Forms Results.

When a form has more than one NPS field, the Results tab adds a find field and a jump menu so you can isolate one survey, plus Collapse all / Expand all. Each report can also be collapsed on its own; the field label and NPS stay visible.

Share a report

On Entries → NPS Survey, use Share report to create a secret link. Anyone with the link can view a live HTML report of the aggregated scores without logging in to WordPress.

  • The page updates as new entries come in. It does not use the Results tab date filter.
  • The link shows scores, mix, buckets, trend, and distribution only. It does not list entries or contact details.
  • Copy the link when you generate or rotate it. The raw URL is not stored, so it cannot be shown again later.
  • Rotate link replaces the URL and invalidates the old one. Disable link turns sharing off.
  • Use Print / Save as PDF on the shared page to save a snapshot from the browser.

Sharing is off until someone with permission to view entries generates a link. Treat the URL like a password: anyone who has it can see the aggregated scores.
